It is now possible for our patient's to fill out and sign their Patient Registration, Financial Policy and HIPAA compliance forms with Pinnacle Internal Medicine Associates prior to the date of their scheduled appointment.

Some of you may be wondering what the HIPAA compliance forms are. HIPAA stands for the Health Insurance Portablility and Accountability Act.The first-ever federal privacy standards to protect patients' medical records and other health information provided to health plans, doctors, hospitals and other health care providers took effect on April 14, 2003. Developed by the Department of Health and Human Services (HHS), these new standards provide patients with access to their medical records and more control over how their personal health information is used and disclosed. They represent a uniform, federal floor of privacy protections for consumers across the country.

Carefully review and fill out the Patient Registration Form below and print it out along with the HIPAA compliance and Financial Policy forms so that you can conveniently bring them to your appointment completed and signed in advance.
